• Launch WordPress and open the Quizzes/Surveys section.
• After that, press the ADD button to create a new form.
• Next, click on the Edit button of the newly created form to open up the Question section.

In each page of the survey form, you can specify one question field and any number of answer fields. Options to create different types of questions are also provided by it namely Open Text, Select One, Select Many, and Multi-line Open Text. In this software, you also get options to create multiple users who will receive surveys. The response of the survey will store in the internal sqlite3 database.

How to create forms using PHP Survey Builder:

• Launch this software and set username and password for this software.
• After that, add users by going to its Users section, who will receive the surveys.
• Next, Move to the Surveys section and press Add Survey button to open the Survey information area.
• Now, add survey title and select a question type (open text, select one, select many, etc.).
• After that, specify the question in the Question text field and add answers according to the selected question type.
• After adding a question, press the Save button and again press the Add Question button to add another question.
• Once the survey form gets completed, press the Save button and visit the Survey section to conduct the survey by pressing the Take Survey button.

You can view the responses of the survey from the Survey section. In it, you also get a view chart option to view survey result in the form of bar graphs. The survey result can also be downloaded in the format of a CSV file.
